Why is “From Another Planet” listed as a separate track?
On this edition, “From Another Planet” is listed as a separate track only on the back cover; “Coc’o Mary” appears there as “Cock-O-Mary”.

What does media VG+ (and better) / sleeve VG+ mean?
VG+ (Very Good Plus) means light signs of wear that barely affect playback; “and better” means parts of the record grade higher. More at Vinyl Grading.
